Residency Verification and Affidavits

Residency Verification (Proof of Address) Requirements

*** NEW FOR 2025-26 ***

District 68 has implemented a new residency verification system to help ensure that students who legally reside within the District's boundaries receive educational services funded by District tax dollars. Residency documents must still be uploaded into your online registration application, but if a student's residence is uncertain, the District may require additional documentation and/or conduct a residency investigation. 

ALL documents must show (1) parent/caregiver’s name, (2) address where the child resides, (3) dated within 60 days of registration, (4) business name & logo, and (5) be from different sources.

Please upload One (1) of the following from Category A AND Two (2) of the following from Category B into your Online Registration Application (OLR): 

 

       Category A        Category B
  • Real estate tax bill
  • Signed lease
  • Mortgage document or payment
  • Warranty Deed (new home purchase within the last 30 days)
  • Military housing letter
  • Section 8 letter
  • Other (to be reviewed by District 68 staff)
  • Gas, electric or water bill
  • Phone bill (not mobile phone)
  • Cable bill
  • Vehicle registration
  • Skokie vehicle sticker payment receipt
  • Bank or credit card statement
  • Other (to be reviewed by District 68 staff)

 

Affidavits

 

You may be required to upload additional notarized documents during the online registration, including:

  • Residency Affidavit for families living with a District 68 resident
  • Residency Affidavit for Divorced/Separated Families

 

If applicable, please print and fill out the below document(s).  These documents will need to be filled out and signed in front of a Notary Public.

Health Forms

Additional forms that may be required to complete your child’s registration are available to download below.  Hard copies are also available at each school office as well as at the Educational Service Center.

 

Physical Exams, Vision and Dental Grade Level Requirements:

  • Physical Exam and immunizations: Required for students within one year prior to entrance into the preschool program, kindergarten or first grade, and sixth grade. The physical exam is also required for students entering an Illinois school for the first time.
  • Vision Exam: Students entering kindergarten or an Illinois school for the first time shall present proof before October 15 of the current school year that the student received an eye examination within one year prior to entry of kindergarten or the school.
  • Dental Exam: All children in kindergarten, second and sixth grade must present proof of having been examined by a licensed dentist before May 15 of the current school year.

Additional Registration Documents

You may be required to upload additional notarized documents during the online registration, including:

  • Residency Affidavit for families living with a District 68 resident
  • Residency Affidavit for divorced/separated families

 

If applicable, you will need to download and complete the required form(s). These form(s) must be signed in the presence of a Notary Public.
